Caregiver Resource Network
• A collaboration of West Michigan organizations
dedicated to providing for the needs and welfare of
family and professional caregivers within the community.

Program for All-Inclusive Care for the Elderly (PACE)
• Delivering all needed medical and supportive services,
the program is able to provide the entire continuum of
care and services to seniors with chronic care needs while
maintaining their independence in their homes for as long
as possible.

Dementia Screening Day
• Free, community-wide, annual screenings for
dementia in partnership with the Greater Michigan
Chapter of the Alzheimer’s Association.

Geriatric Education Center of Michigan (GECM)
• A grant administered by Michigan State University, this collaborative project offers education and training for health professionals to enhance quality and availability of health care for older adult.
